Individual Support Packages

All new Victorian funding provided by the Department of Human Services is now allocated to individuals. These are referred to as ISP's.

How to apply for an ISP?

Community Living & Respite Services Inc. has an intake/planning coordination who will assist you to register for ongoing supports with the Department of Human Services (DHS). DHS has a register called the Disability Support Register (DSR). If you, or the person you care for, has a need for ongoing supports, you may like to contact our intake/planning coordinator on (03) 5480 2388 or the DHS intake team on (03) 5434 5555.

Examples of ISP's

Some people use their ISP to purchase respite services. Others use the funding for in home care, access to the community, assistance with daily living. The range of options vary according to individual need..
