At times in our lives things can be overwhelming and complex, especially as we get older or have a disability, or if we are caring for loved ones who are ill or have a disability.

Who do I contact?, What services are available to me? or How will I ever afford this equipment?

The role of a Case Manager is to assist people with complex needs to access services and resources and liaise and negotiate with organizations to ensure your needs are meet.

Together we work to identify needs, goals and strategies.

Case Management can be short term or long term depending on your needs.

Community Living & Respite Inc. is currently providing Case Management through brokerage organizations.
